In order to provide our patients with Medi-Cal (also called Medicaid, Santa Clara Family Healthplan, Blue Cross Medicaid) timely appointments, we have developed this system.

• All patients with these plans must request an appointment through this webpage. Do not call the office.
• The form must be filled out completely: 
  • Patient name
  • Patient Medicaid ID number
  • Patient date of birth
  • Reason for exam
  • We require a primary care doctor referral for children 5.
• Any requests that do not have all the above information will not be accepted.
• If you are not eligible for an exam with your insurance, the request will not be accepted.
• All patients will be placed on our waitlist only.
